While you’re making your birth plan, we suggest you look into the philosophies of several childbirth preparation techniques. To be effective, all of these techniques require you to start practicing their methods before the third trimester.

Three of these techniques are ASPO/Lamaze, the Bradley method, and HypnoBirthing. All of them teach you how to be an active participant in your labor, how your body works during labor, and recommend exercises that may decrease the pain and shorten labor. The goal for each of these methods is that you have a personal, natural, and joyful birth experience. We recommend that you explore these Childbirth Preparation techniques to see if you feel they fit your hopes for your birth experience and will make your birth journey easier.

Technique
Make a birth plan by writing down the following preferences, realizing that they are your preferences and if complications occur your baby and your health are the major concern:

• Place of delivery: hospital, birthing center, or home birth

• Professional for prenatal care and delivery: doctor or midwife

• Labor Doula: yes or no

• Childbirth preparation method: ASPO/Lamaze, Bradley, Hypno-Birthing, other

• Prefer pain medication: yes or no

• Prefer vaginal birth after a cesarean section: yes or no

• Prefer not to have an episiotomy: yes or no

• Planning to breast-feed: yes or no

• Do you want your male baby circumcised: yes or no

Note: You may not feel ready to decide on all of these factors. And, as you go through your pregnancy, you may change your mind about your initial preferences. You should always feel you have the right to do so.

Thyroid and Pregnancy. In pregnancy must take into account two factors, the thyroid of the mother and the child’s thyroid. Everyone has their own personality and requires its own attention.

Thyroid of the child.

In relation to the thyroid of children have to remember the “rule of three”

After three weeks of conception begins to form the outline of what must be the child’s thyroid, the woman at this time often do not know yet who is pregnant, from the third month of conception, the fetal thyroid is already able to accumulate iodine, operate and produce its own hormones, three weeks after birth the newborn baby has used hormones that could come from their mother, have exhausted their deposits and began to work connected with the pituitary gland.

During pregnancy, the first embryo and fetus after they are well protected. They have two possibilities of using thyroid hormones: those that he makes and those of his mother passing the placenta. If your mother has a severe hypothyroidism, the child uses the hormones it produces, if the child has a thyroid agenesis, or congenital lack of thyroid, also has no problem, use the hormones that supplies your mother. In both cases the child is born perfectly normal in their development.

The thyroid problem during pregnancy occurs when there is a lack of iodine to produce thyroid hormones. In these circumstances it works well or the thyroid of the mother or the fetus, and then there are problems.

Mother’s thyroid. Changes induced by pregnancy.

Although she is a woman you absolutely normal pregnancy for thyroid is a major impact. But do not worry you are prepared for that and more. That gender equality is nonsense, women are physiologically better equipped than men.

Pregnancy is accompanied by the influence of a number of factors specific to this condition which together represent a major stimulus of the thyroid of the pregnant woman.

The first factor, which influences all in the first quarter, thyroid stimulating hormone produced in the placenta, chorionic gonadotropin Lets talk a little about it. Production begins immediately with the design, the 2 to 3 days. It is this hormone that is detected in blood or urine and is the basis of pregnancy tests. However, chorionic gonadotropin is very similar to TSH and may stimulate the thyroid. Approximately 18% of pregnant women during the first quarter a slight stimulation of the thyroid that goes unnoticed among the other pregnant women feel discomfort. There may be a slight elevation of T4 and a suppression of TSH in the first quarter is normal and should not be confused with hyperthyroidism. In the second and third quarter down chorionic gonadotropin levels and the picture tends to subside.

Well, if there is an increase of thyroid hormones in blood and inhibition of TSH, a gynecologist and endocrinologist have the right to think there may be a hyperfunctional status. But there are data that can help make a difference: first normal antithyroid antibodies and especially the ultrasound is normal in this false hyperthyroidism might be called the first trimester of pregnancy. In hyperthyroidism there is an increase authentic vascularization in the thyroid gland appears as a low echogenicity, the study eco-color doppler is very apparent increased vascularity, in the false hyperthyroidism thyroid ultrasound is absolutely normal.

In the second and third quarter are also factors that alter thyroid function, but are otherwise. Although not entirely clear, this problem could be implicated themselves female hormones, primarily estrogen. We discussed in the opening chapters of Physiology which circulating thyroid hormones in the blood largely bound to a protein called TBG (thyroxine binding globulin Bindig Globulin or thyroxine or Conveyor), well in pregnancy figures TBG soar, between 16-20 weeks of gestation the number of TBG in blood are doubled. In these circumstances the levels of T4 and T3 in the blood are altered and can give the false impression of a functional impairment. But this issue is resolved, because the T4-Free is unchanged and remains normal. Simplify and summarize:

In pregnant by changes in hormonal status can be seen discrete changes in the levels of T4, T3 and TSH. -Free T4 is not altered. Conventional ultrasound and echo-color doppler (vascularization), are normal.

For the reasons we have cited above the thyroid of the mother during pregnancy is forced. It is estimated that the thyroid of the pregnant woman produces between 30% and 50% more than in normal thyroxine. To maintain the level of T4-Free stabilized blood must increase its production rate and also the mother to the fetus transfer a portion of their hormones, not much, but enough to maintain normal growth if the fetus had problems with your thyroid .

In pregnancy the mother’s thyroid can grow a little. But perhaps not always in a 10-15% of cases. This confirmed by ultrasound. But it’s a very discreet and you can later return to their normal size.

Regulation of thyroid function in pregnant women with iodine deficiency.

Under normal conditions, as we previously indicated, the moderate exertion requesting the thyroid is fine, but if there is a deficiency in iodine, the situation is different. The thyroid of the mother is insufficient to produce the quantity of hormone needed, because it has enough iodine without material may not be can not build a house. The mother’s TSH is elevated and the thyroid of the mother increases. The child feels the same and also may be born with a small goiter, apart from the problems of development that could have had.

The iodine requirements of a normal woman is 150 micrograms per day, pregnant women need 200 micrograms. Iodine supplementation in the form of single dose “shock” in the first two months of pregnancy or compounds included in multivitamin and polimineral type of currently used can solve this problem.

Here are 11 Tips for the First Stage of Labor:

- Have your partner help you try different positions to minimize the discomfort and to promote relaxing.

- Try to keep moving by getting up to go to the bathroom, walking around the room, and sitting in a chair. Muscles tense when they Ye held in the same position and will use up oxygen and calories and increase pain.

- Try warm showers and massage.

- Start the breathing and relaxation techniques you learned in your childbirth classes.

- Empty your bladder frequently.

- Sip water or suck on ice chips. You may be allowed Popsicles.

- Panting helps keep you from pushing too early, and breathing exercises distract your mind from the intense contractions.

- Change positions frequently. Try to roll from side to side, sit, or get on your hands and knees on the bed. These changes in position will help relax your muscles and regulate your breathing so you and the baby get more oxygen.

- Put ice packs on your back to relax tense muscles and numb the back pain.

- Keep close contact with your partner or coach. At this point you may want to be left alone to internalize your feelings and pain. Don’t let this happen! Prearrange a signal to let your spouse or coach know you’re feeling this way. Tell yourself to keep listening to your coach. This is probably the most critical time for your coach to offer encouragement.

- Request something for pain. Talk with your doctor or midwife about this before you go into labor. Know what’s available, the degree of relief you can expect, and when and how the medication is given. This is often the time when women request something for pain if they haven’t already had an epidural.

Looking After Your Body When You Are Pregnant. Everyone knows that it is important to look after yourself when you are pregnant but it is not always obvious exactly what should be done. It is worth talking to your doctor or midwife about it as soon as you get pregnant. They will explain to you what foods you should and should not eat and how to have a healthy balanced diet so that you get all of the nutrients you need for the baby. They will be able to let you know if you should be having any supplements as well. It is very important to make sure that you take folic acid at the very start of your pregnancy if not before and so this is a good reason to see your doctor or midwife as soon as possible. You may also need to take a n iron supplement if you are having a multiple birth or if you are having a c-section. The medical staff will know exactly what you should be doing and so make sure you listen and take their advice. It is also important to remember that you need to look after yourself as well as the needs of your baby as you will need to be fit and healthy so that you can look after the baby when it is born.

Keeping Fit in Pregnancy. While you are pregnant it is important to keep at a good level of fitness if you can. You may need a lot of stamina for the birth and also a lot of energy and if you are fitter and stronger then this should be easier. It is not sensible to start doing a lot more exercise once pregnant than you were doing before hand, this might mean that you strain muscles and make yourself overly tired.

However, talking a walk each day, doing some yoga and maybe some pregnancy fitness DVD’s should be plenty to keep you busy and keep you fit. If you are used to being far more active then there is no harm in doing more but be sensible. Certain activities carry risks, such as rock climbing or horse riding. If you fall, you could do enough damage to harm the baby or even lose it and so you need to think about whether it is worth doing these things or not. If you are in any doubt as to how much exercise you should be doing then talk to your doctor or midwife about it. If you tell them about your pre pregnancy fitness routine, they will let you know whether it is wise to continue with that or not.
